Step one:

Start with a white canvas. Make a new layer set titled Icon.
Choose your polygon tool and set sides to 3 and check anti-aliased.
In a new layer draw a #006CBF polygon using your polygon tool. ( width: 62px x height: 60px)
Add the following layer styles:

Inner Glow

Gradient Overlay

Stroke
